Theresa died August 11, 2005 at the Lantern Park Nursing and Rehab Center in Coralville at the age of 98. Funeral services will be 10 a.m. Tuesday August 16, 2005 at St Anthony's Catholic Church. Visitation will be held from 4 to 8 p.m. Monday with a 7 p.m. Rosary at the funeral home.

Theresa was born in Des Moines where She lived most of her life. She worked for A biscuit company in her early years. Theresa was a member of the American Legion Auxiliary and a past member of St. Anthony's Catholic Church.

Theresa enjoyed playing bingo, going out to eat and she loved spending time with her family and friends.

She is survived by a daughter, Lucille (Chuck) Sharp of Coralville; a twin sister, Rose Mazzie of Des Moines; a brother, Ralph Costanzo of Des Moines; and one grandson, Charley Sharp of San Diego, CA; as well as nieces, nephews and cousins. She was preceded in death by her husband, Lee Chach; her parents, Rose and Peter Angelo Costanzo; her sister, Angie Gillotti; and her brother, Tom Costanzo.
